I understand that permanent jewelry involves a small jump ring being welded closed while the jewelry is worn on the body. I voluntarily consent to receiving permanent jewelry and/or engraving services from Eterna Permanent Jewelry & Engraving.

I understand a small welding spark and flash may occur during the process. I understand there is minimal risk of irritation, burns, snagging, breakage, or allergic reaction. I voluntarily consent to receiving services from Eterna Permanent Jewelry & Engraving.

Medical Acknowledgment

I confirm that I do not have a pacemaker or implanted electrical device. I confirm that I do not have epilepsy or light sensitivity. I confirm that I do not have skin conditions or infections near the jewelry area.

Liability Release, Care & Removal

I release and hold harmless Eterna Permanent Jewelry & Engraving from liability relating to allergic reactions, breakage, snagging, improper aftercare, or injuries resulting from misuse. Permanent jewelry may need to be removed for MRI scans, medical procedures, work regulations, or emergencies and can be removed with scissors or wire cutters.

Refund & Return Policy

All permanent jewelry and engraved items are final sale and non-refundable. Each piece is custom measured, cut, fitted, welded, and/or engraved specifically for the client. Returns, exchanges, or refunds are not accepted once the service has been completed. If the weld breaks within 7 days of purchase, we will reweld it free of charge. After 7 days, rewelding is available for a $10 fee.

Photo & Video Consent
